Important - please read carefully Print this form and use it for your entry or entries to the contest. Choose a font size using the control on your browser menu to make sure the table fits correctly on your screen. Send it with your festival payment form to Peter Wheat at the address provided on it. All competitors, playing solo, or in duos or groups must each fill in the contest entry form, and pay the contest entry fee. Details of the group entry must be made by the group leader only. He should name all members of the group. Every competitor pays just the one contest fee, but may enter as many sections of the contest as they wish.

Enter "Improvisation" as an alternative to named pieces as appropriate

Every contestant should fill in one festival entry form with his name and address and pay his own fee/s.

A contestant who enters only solo classes, but no group classes need fill in only one contest form (this one).

For GROUPS - When this form is used to enter details of a group or groups, the group leader should name each member and indicate the group or groups in which he will play. e.g. Colin Powell [Duo and Band], Fred Green [Duo and Band], etc.

Please use one form for each group entry entered.
